The ATA Carnet General List Form and Continuation Sheets

The green ATA Carnet General List Form is the reverse side of the ATA Carnet cover.

The columns to be completed by the applicant online are: Item No., Trade Description of goods and marks and numbers, if any, Number of Pieces, Weight or Volume, Value, Country of Origin.

The General List form contains 50 lines of data. Continuation sheets are used for more than 50 lines of data. We do not charge for additional sheets.  A QuickList service is available to help import a list of items.
